Melanotan-1 (afamelanotide) is a synthetic analogue of alpha-melanocyte stimulating hormone (α-MSH) with selective activity at the melanocortin-1 receptor (MC1R). It was developed at the same University of Arizona programme that produced Melanotan-2, but was taken forward as a pharmaceutical candidate for photoprotection rather than for its sexual function effects, because MC1R selectivity avoids the central nervous system activity at MC3R and MC4R. It received European Medicines Agency approval in 2014 as Scenesse (implant formulation) for erythropoietic protoporphyria — a rare genetic disorder causing extreme photosensitivity.

What the research covers

MC1R stimulation increases melanogenesis — the production of eumelanin (dark pigment) in melanocytes. This produces tanning and provides photoprotection by absorbing and scattering UV radiation. In the protoporphyria indication, this photoprotection allows patients who would otherwise be unable to tolerate sunlight to spend time outdoors. Research outside EPP has examined afamelanotide in polymorphous light eruption (a common photosensitivity condition) and in vitiligo, with some evidence of repigmentation. There is also interest in its interaction with the immune system through MC1R on immune cells.
