PT-141 (bremelanotide) is a cyclic heptapeptide derived from Melanotan-2, with a structural modification that removes the erection-inducing activity while retaining central melanocortin receptor activity. It was developed specifically as a research compound for sexual dysfunction and later received FDA approval in 2019 as Vyleesi for hypoactive sexual desire disorder (HSDD) in premenopausal women — making it one of the few compounds in this space with a current marketing authorisation, though not in the format supplied here.

What the research covers

PT-141 acts centrally on MC3R and MC4R receptors in the hypothalamus rather than on the vascular system. This distinguishes its mechanism from PDE5 inhibitors (sildenafil, tadalafil) which work peripherally on blood vessel smooth muscle. The central mechanism means it affects desire and arousal rather than simply penile blood flow. Clinical trials in both men and women demonstrated statistically significant effects on desire and sexual satisfaction endpoints. The male HSDD data was not pursued to approval, though the mechanism is the same.
