SereneQ SS-31

SS-31 (elamipretide) is a synthetic aromatic-cationic tetrapeptide that selectively concentrates in the inner mitochondrial membrane, where it binds to cardiolipin — a phospholipid that is essentially unique to mitochondria and critical to the organisation and function of the electron transport chain. Cardiolipin content and composition changes with ageing and disease, and these changes are linked to reduced electron transport chain efficiency and increased reactive oxygen species (ROS) production. SS-31 is thought to stabilise the cardiolipin-cytochrome c interaction, which governs the efficiency of electron transfer through Complex I and Complex III.

What the research covers

The majority of published work is in models of ischaemia-reperfusion injury, particularly cardiac. SS-31 has consistently reduced infarct size and improved ventricular function in animal studies of acute MI. Renal ischaemia-reperfusion studies showed similar protective effects. In skeletal muscle, age-related decline in mitochondrial function and exercise capacity has been studied in both animal models and a small Phase 2 human trial in older adults, which showed improvements in muscle ATP synthesis rate measured by phosphorus-31 MRS. The drug is in late-stage clinical development (as MTP-131) for primary mitochondrial myopathy and Barth syndrome.

Where the evidence sits

The most robust human data comes from the muscle ATP synthesis study and the ongoing rare disease programme. It does not hold a general medicines licence. Supplied strictly as a laboratory research chemical.
